- Utilize monitoring, assessment, planning, intervention and critical thinking skills to care for acute care patient populations in the home.
- Conduct home and audio / visual patient care visits as directed by the care team and / or protocol.
- Ensure all interventions in the patient's care plan are implemented efficiently with a patient centered approach.
- Adhere to the clinical protocols and be willing to obtain the necessary training to provide care within the context of the providing evidence-based care.
- Promote professional practice and a culture of safety; willingness to engage in process improvement efforts. Requirements
- Associate Degree required
- 2 years of Registered Nurse experience in an acute care setting. Home health experience preferred.
- Valid driver's license, reliable vehicle and proof of vehicle liability insurance.
- Must be able to communicate effectively in English.
- Demonstration of core competencies will be completed within 90 days..
- Certified CPR and negative TB skin test or chest x-ray
- Ability to work on weekends, nights and holidays
- Reliable transportation
- Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the mobility of the client.
