Hours : 32hr, Day shift position

Job Summary

Under the direction of Practice Providers, and Sr. Practice Admin, the incumbent is responsible for assisting with patient care.

Responsible for streamlining effective processes and providing process and quality improvement methods to enhance efficiency, productivity, and the patient experience.

Adapt to quickly changing priorities and needs while fostering teamwork within the department. Along with the Sr Practice Manager interview, hire, train, and supervise staff.

Monitors and ensures work is performed in conformance with established hospital and departmental policies and procedures.

Must learn and implement new standards and processes and participate in and direct customer service and patient satisfaction initiatives.

Essential Functions

  • Provide mentoring and support
  • Provide coverage when needed at other locations
  • Prepare staffing schedules
  • Manage clinic schedules
  • Manage clinic supplies
  • Welcome, check-in, and prepare patients for their visit
  • Complies with all HIPAA regulations with regards to patient privacy
  • Verify patient information for completion and accuracy
  • Must be comfortable wearing the proper protective equipment during work hours
  • Process and communicate patient testing results for pre-procedures
  • Along with the Sr Practice Manager interview, hire, train, and supervise staff
  • Monitors and ensures work is performed in conformance with established hospital and departmental policies and procedures
  • Must learn and implement new standards and processes and participate in and direct customer service and patient satisfaction initiatives
  • Promotes a culture of safety within the organization
  • Complies with established departmental policies, procedures, and objectives
  • Attends variety of meetings, conferences, training and seminars as required or directed.
  • Demonstrates use of quality improvement in daily operations
  • Respects diverse views and approaches, and contributes in maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors.
  • Leads / participates in departmental or organizational meetings as required
  • Develop and maintain excellent relationships with community providers
  • Build a listing of available local resources
  • Track and identify service gaps in access to care for school age population, and primary usage of care coordination
  • Provide "hands on" case management to families accessing school based counseling and / or outside services including but not limited to insurance applications, fuel assistance, food banks, primary care, etc.
  • Builds collaborative and mutually respectful relationships with school personnel and community organizations
  • Advocates for the resources necessary to increase access to health services for students and families they support
  • Acts as liaison between organizations to ensure that resources are used and shared in ways that maximize efficiency and cost effectiveness
  • Collects, evaluates, and reports behavioral health outcomes and utilization data
  • Day-to-day monitoring and problem solving various tasks and activities as related to student and family needs upon referral from Guidance Department
  • Connecting students and families to a range of school and community resources and provides help to ensure that those resources are accessed
  • Collaborating with school based behavioral health clinician, school staff, and organization / agency representatives to develop systems for ensuring coordination of services for students in need of support
  • Orients patients and their families with an initial meeting that explains role of care coordinator, involvement in care plans, provides educational materials

Qualifications

Job Requirements

Minimum Education

High School diploma or equivalent preferred.

Minimum Work Experience

Previous medical office experience preferred.

Additional Requirements

  • Excellent written and verbal skills.
  • Excellent customer service skills.
  • Able to prioritize tasks in a busy environment.
  • General comfort level with computers / technology and ability to learn programs as needed. Knowledge of Electronic Medical Records Preferred and word processing skills required.
  • Ability to maintain strict compliance with HIPAA regulations is required.

Exerts up to 20 pounds of force occasionally, and / or up to 10 pounds of force frequently, and / or a negligible amount of force constantly to move objects.
