Licensed Clinical Social Worker (LCSW) SARP / WII

Position Duties and Responsibilities :

  • Independently function as the primary mental health therapist, providing assessment and therapeutic interventions in outpatient settings utilizing clinical modalities exercising mature professional judgment and utilizing a wide range of social work skills to include individual, family, couples, and group counseling to assist patients and their families in dealing with acute, routine, and chronic mental health conditions.
  • Practice as an interdependent member of the health team and provide important components of primary health care through direct social work services, consultation, collaboration, referral, teaching, and advocacy.
  • Provide direct and indirect services to both inpatients and outpatients in accordance with social work standards of practice.
  • Assess and treat outpatients in individual, group, couples, and family modalities exercising professional judgment, evidenced-based therapeutic interventions and clinical practice guidelines where appropriate using a wide range of social work skills to include individual and family counseling to assist patients and their families in dealing with chronic and acute diseases / injuries.
  • Conduct psychosocial assessments to determine patient needs and resources (both family and community support). Provide counseling to patient and family in matters directly related to patient limitations, adjustment to medical condition, and ongoing treatment.

Develop and implement discharge plans, follow-up care, and transfers to other health care facilities (e.g., nursing homes, rehabilitation hospitals, and VA facilities).

  • Plan and maintain referral and coordination services with civilian health and social service agencies to provide optimal patient care.
  • Provide consultation services to medical, nursing, and ancillary hospital staff regarding psychosocial issues, discharge plans, and follow-up care for patients and families.
  • Provide crisis intervention services.
  • Respond with various media, to appropriate military and community requests for lectures and presentations. Take the initiative in seeking out opportunities to present programs to meet the needs of service members and their families.
  • Develop and maintain working relationships with community resources. Coordinate with teachers, lawyers, physicians, and representatives of their service disciplines for the benefit of the service member and families.

Take initiative in identifying and assessing the needs of the military community and organize responses to address those needs.

  • Provide behavioral health care for all enrollees in the patient centered Medical Home Port (MHP) program using evidenced-based interventions and clinical practice guidelines where appropriate for treating behavioral health problems.
  • Perform limited administrative duties that include maintaining statistical records of the HCWs clinical workload, participating in education programs, and participating in clinical staff Performance Improvement Functions and Process Action Teams, as prescribed by the Commander.
  • Participate in monthly meetings to review and evaluate the care provided to patients, identify opportunities to improve the care delivered, and recommend corrective action when problems exist.
  • Participate in the provision of monthly in-service training to non-healthcare practitioner members of the clinical and administrative staff on subjects germane to social work services.

This shall include responsibility for developing and implementing psycho educational programs, community relations, information and referral.

Minimum Qualifications :

Must have graduated from an Masters of Social Work program accredited by the Council of Social Work Education (CSWE) with a clinical focus.

The incumbent must be able to be fully credentialed at the Medical Treatment Facility (MTF) in the provision of individual, group, marital, couples, family therapy and crisis intervention utilizing evidenced based psychotherapy methods.

  • Minimum of one year of experience as a LCSW.
  • Current, full, active, and unrestricted license to practice as a Licensed Clinical Social Worker.
  • Provide two (2) letters of recommendation written within the last two (2) years attesting to the HCW’s clinical skills.

Letters of recommendation must be from practicing physicians, psychiatrist, psychologist, or licensed clinical social work (LCSW) supervisors who have personally observed and evaluated the HCW and who can attest to the HCW’s clinical and professional knowledge, skills, competencies, patient rapport, training and abilities.

Reference letters must include name, title, phone number, date of reference, address and signature of the individual providing reference.

Must be a US Citizen.
