Job Title : LCSW Location : San Mateo, CA Duration : 13 Weeks Pay Rate : $3,072 - $3,152 / week Shift : Day 5x8-Hour (08 : 00 - 16 : 30) Duties : The Clinical Social Worker provides crisis intervention, short term counseling, information and referrals for patients and their families, ranging in age from infants through geriatrics.
Provides coordination of complex discharge planning needs and concrete service needs of patients. Leads educational and support groups.
Provides consultation related to psychosocial aspects of care as a member of the interdisciplinary treatment team. Serves as an expert in community resource programs.
Is a liaison to community referral agencies, both public and private. Participates as a member of the Bioethics Committee.
The Clinical Social Worker coordinates all aspects of social barriers to care with the case manager and the interdisciplinary team, to ensure effective, high quality, timely and cost-efficient results.
