Job Overview : Registered Nurse Assessor
We are seeking a Registered Nurse Assessor for a hybrid role conducting home assessments and telehealth evaluations. This position requires a blend of in-field and remote work to assess and support patient needs.
Location : Queens, NY
Compensation : $52 / hour
Schedule : Monday to Friday, (3 days dedicated to home assessments and 2 days to telehealth evaluations
Responsibilities as the Registered Nurse Assessor :
- Conduct initial in-home pre-evaluations and develop personalized care plans.
- Review and document consumer medical information.
- Discuss health care options with consumers.
- Communicate with Call Center for managed care enrollment.
- Collaborate with care team members to improve service delivery.
- Collect quality review data and maintain accurate records.
- Identify opportunities for health promotion and illness prevention.
- Perform other duties as assigned by management. Qualifications for the Registered Nurse Assessor :
- 2+ years of recent experience in a home setting (e.g., CHHA, LHCSA, MLTC).
- Experience with assessments similar to UAS.
- Strong organizational, interpersonal, and communication skills.
- Ability to work independently and manage multiple tasks.
- Preferred : Experience with UAS-NY assessments and geriatric care.
- Proficiency with technology and electronic health records.
