Job Summary

The Remote Clinical Pharmacist is responsible for providing telephonic consultations to assess prescription claims data and deliver clinical recommendations in line with Medication Therapy Management protocols.

This role ensures the optimization of medication regimens, offers patient education, and maintains compliance with HIPAA regulations.

This role will allow the pharmacist to interact with members at a Wellness center one day a week and telecommute the rest of the week (currently not going into the Wellness center but may require in future).

Pay : $53 73 an hour based on experience

Hours : Full Time - Monday-Friday. Flexibility is required for rotating shifts between 10 : 00 AM and 8 : 00 PM CST. Extended hours and occasional weekends may be required as needed.

Job Duties :

• Conduct telephonic outreach to schedule pharmacist consultations with members enrolled in clinical programs supported by the Clinical Engagement Services Center
• Utilize prescription claims data to generate clinical recommendations according to Medication Therapy Management and Retrospective Drug Utilization Review program protocols providing clinical recommendations pertaining to, but not limited to, gaps in care, high risk medications, compliance and adherence, drug interactions, therapeutic substitution, and generic substitution
• Evaluate patient medication regimens for unnecessary medications, inappropriate dosing, and adverse drug reactions
• Provide patient education on medications and relevant disease states
• Accurately document patient care notes and clinical recommendations in a clear, professional, and concise manner
• Provide monthly presentations at Wellness Center on pertinent health topics
• Adhere to HIPAA regulations and company policies to ensure the privacy and confidentiality of patient information
• Visit providers and / or long-term care facilities to complete member Medication Therapy Management consultations
• Stay updated on emerging clinical trends and actively participate in departmental meetings to improve clinical programs

Requirements

• Bachelor’s Degree in Pharmacy or PharmD
• Active, unrestricted Pharmacist license in the state of Utah
• Must live within a commutable distance of the Sandy, Utah regional area
• Strong knowledge of disease management guidelines and the 5 Core Elements of Medication Therapy Management services (Medication Therapy Review, Personal Medication List, Medication-Related Action Plan, Interventions and Referrals, Documentation of Services and Follow-up)
• Proficiency in navigating Medication Therapy Management platforms, databases, and applications with competent computer skills
• A reliable high-speed internet connection (the faster the better)
• Ability to pass a background check and drug test
• Private, quiet, and distraction-free workspace in a room with a closed-door

Highly Preferred

• Large corporation experience
• Health plan / managed care / healthcare industry experience
• PharmD
• Current and active Certified Diabetes Care and Education Specialist, Board Certified-Advanced Diabetes Management, ASHP Diabetes Management Certificate, or APhA Patient Centered Diabetes Care Certificate
• Bilingual (fluent in medical terminology) in Cantonese, Korean, Mandarin, Russian, Spanish, or Vietnamese
• Additional Pharmacist License in good standing in Alabama, Idaho, Kentucky, Louisiana, Maryland, Minnesota, Oregon, Tennessee, or Vermont
• 2+ years Pharmacy work experience in retail / community pharmacy and / or Pharmacy Benefit Manager, but will consider new graduate experientials
• Prior call center experience
• ACPE MTM Specialist
• Certified Geriatric Specialist
• Board Certified Pharmacist
