First Choice Home Health (Jet Health, Inc.) is looking for a full time / part time or PRN Licensed Practical Nurses (LPN / LVN) to join our dedicated team to provide community-based care for our patients currently receiving home health services in the Boise, ID (Ada County) and / or Nampa, ID (Canyon County) metro area.
The primary function of the LPN is to provide nursing care, under the supervision of a registered nurse, for patients of all ages in their place of residence.
The LPN will coordinate care with the interdisciplinary group, patient / family, and referring agency to ensure the best patient outcomes.
Requirements :
- Provide nursing care to patients of all ages in their homes under the supervision of a registered nurse.
- Coordinate care with the interdisciplinary group, patient / family, and referring agency.
- Assist in the development and implementation of patient care plans.
- Monitor patient health and report any changes to the supervising RN.
- Administer medications and treatments as prescribed.
- Educate patients and their families on health care needs, conditions, and options.
- Maintain accurate and timely documentation of patient care and progress.
Job Qualifications :
- Education : Graduate of an accredited School of Vocational / Practical Nursing.
- Licensure : Current State license as a Licensed Vocational / Practical Nurse, or a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC) if practicing in a state member of the NLC.
Must have a current driver's license.
- Experience : Two years’ experience as a Licensed Vocational / Practical Nurse in a clinical care setting. Home health experience highly preferred.
- EMR system : HCHB experience highly preferred
- Skills : Proficiency in nursing skills as defined by generally accepted standards of practice. Good interpersonal skills.
- Transportation : Reliable transportation and valid, current auto liability insurance.
Environmental and Working Conditions :
- Work in patients' homes in various conditions.
- Possible exposure to blood, bodily fluids, and infectious diseases.
- Ability to work a flexible schedule and travel locally.
- Some exposure to unpleasant weather conditions.
Physical and Mental Effort :
- Prolonged standing and walking required.
- Ability to lift up to 50 lbs and move patients.
- Requires working under some stressful conditions to meet deadlines and patient needs.
- Ability to make quick decisions and acquire necessary resources.
- Must meet patient / family individualized psychosocial needs.
- Requires hand-eye coordination and manual dexterity.
