License : State License Verification RequiredCertifications : Certified Occupational Therapy Assistant (COTA)Must-Have : - Resume and License Verification- Prescreening Document- Covid Test 72 hours prior to start date with results issued 48 hours prior to start dateJob Description : - Fill out and attach the prescreening document upon submitting an application.
Ensure the resume and license verifications are attached.- Provide three references.- Complete the following background checks : - County Criminal Check (7 years for all counties lived and worked) - Federal Criminal Check (7 years or more) - Multi-State Criminal Search (7 years or more) - Nationwide Sex Offender Search (7 years or more) - HHS / OIG Search - GSA Search - Social Security Number Trace - 1 Step PPD or Quantiferon or Proof of Positive PPD and Chest X-Ray - SAM Search - CORI Massachusetts Background Check- Complete 8 hours of Dementia Training.
