About The Position
Job Title : Social Worker Job Specialty : Health Sciences - Social Worker Job Duration : Full Time Shift : Days, 8 : 00 AM - 4 : 30 PM Guaranteed Hours : Experience : At least 2 years in acute care License : Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) as required by the state Certifications : CCM preferred within 18 months of eligibility Must-Have : Must have a Master's degree in Social Work (MSW) or Master of Social Services (MSS) from an accredited graduate school Job Description : We seek experienced Social Workers with a passion for care coordination and acute care experience.
- Engage in a Care Management Triad Team Model, collaborating with RN Case Managers and Utilization Management specialists.
- Focus on resolving psychosocial barriers and supporting discharge planning.- Provide compassionate care in a dynamic environment with opportunities for professional growth.
