About The Position

LTC or Rehab experience required License : Texas State License required Certifications : Not specified Must-Have : - 1 year of PT experience- LTC or Rehab experience- Texas State License Job Description : Effectively screen, evaluate, and rehabilitate patients with physical functioning disorders and / or wound care.
- Develop appropriate care plans following all regulatory and clinical practice standards.- Utilize standardized assessments and evidence-based practices to support clinical interventions.
- Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, and use of supportive and assistive devices.
- Provide wound care and adjust treatments according to patient progress.- Evaluate treatment outcomes for effectiveness and modify services as necessary.
- Provide consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
- Communicate effectively with supervisors and health team members regarding patient progress, barriers, and treatment plans.
- Provide comprehensive discharge summaries of services provided, adhering to regulatory and clinical requirements.- Document all required information in the electronic documentation system and maintain patient records according to company policies.
- Accurately record treatment minutes for services provided, adhering to all regulatory and clinical requirements.- Participate in required meetings, including patient care conferences, utilization review meetings, rehabilitation meetings, etc.
- Engage in facility and company-required trainings, in-services, and conferences.- Obtain necessary DME or supplies per regulatory and clinical practice requirements.
- Supervise and support physical therapist assistants, rehabilitation aides, and / or students.- Adhere to APTA and State Required practice acts, codes of conduct, and ethics during professional practice as a Physical Therapist.
