The Registered Nurse in Home and Community BasedServices provides and directs provisions of nursing and personalcare to patients in their homes as prescribed by the physician oras authorized by the state / program and in compliance withapplicable laws, regulations, and agency policies.

The RegisteredNurse coordinates the plan of care with other health careprofessionals involved in care and helps to achieve and maintaincontinuity of patient care by planning and exchanging informationwith physician, agency personnel, patient, family, case manager andother community resources.

• Provides high quality clinical services within the scopeof practice and within infection control standards, in accordancewith the plan of care / service plan, and in coordination with othermembers of the patient / client’s care team from admit throughdischarge.
• Completes clinical nursing assessments inaccordance with federal and / or state program requirements and asrequired by payer.
• Ensures thepatient / client’s eligibility and medical necessity for services asdefined by payer source and agency policy.
• Developsand revises individualized plans of care and / or service plansaccording to federal and / or state program requirements with othercommunity providers.
• Ensuresplan of care frequency and duration meets the needs of the patientand initiates plan of care revisions as needed with physicianapproval.

Experience Desired A minimum of one year experienceas an RN preferred License Requirements

• Must have current RN licensure in state ofpractice.
• Current driver's license, vehicleinsurance and access to a dependable vehicle or publictransportation.
• Current CPR certificationrequired.

State SpecificRequirements

LA : At a minimum, one year of clinical experience as aRegistered Nurse. One year of clinical experience as a RegisteredNurse may be waived for a Registered Nurse with recent clinicalexperience as an LPN.

RN licensure must have norestrictions

AR, AZ, CT, D.C., FL, KY, MA, MD,MO, NY, NC, NH, NJ, OH, PA, TN, TX, VA, WI : No other state specificrequirements.
