Shift : Monday - Friday 8 : 30am - 5 : 00pmSign on Bonus offered to qualified candidates!The Home Health RN is responsible for the development and implementation of home care services for program participants to include the coordination of durable medical equipment and other health care services.

Requirements : * VA Registered Nurse License (RN) required* BLS required* 1 year nursing experience preferredWith locations in Portsmouth and Norfolk, Sentara PACE recognizes that older people often have medical problems that last for long periods of time and require services from several different sources.

Of great concern is the fact that, as many people age, they become increasingly isolated from the support, companionship and resources of the community at large.

Sentara PACE is that important link, assuring participants receive the care they need, when they need it.This program is a comprehensive health care and supportive services program for frail seniors who wish to remain in their homes and community.

It is an alternative for those who are eligible for a nursing facility but would prefer to remain at home.The program provides total care for participants, including comprehensive medical and rehabilitative services, in-home services and transportation.

Sentara PACE is offered for residents of Norfolk, Virginia Beach, Chesapeake, Suffolk, Portsmouth, Surry, Isle of Wight and the city of Franklin.
