• gov / manage-loans / forgiveness-cancellation / public-service) . Typical Duties : + Perform assessments, diagnosis, treatment, and prevention of mental illness, emotional, and other behavioral disturbances;
• Assess patient's needs, situations, strengths, and support networks to determine their goals; + Collaborate with Family and Clinic Service providers on medication treatment;
• Create treatment plans; + Help patients adjust to changes and challenges in their lives; + Monitor patient progress throughout their counselling plan;
• Provide population-based education, prevention, and early intervention with patients and participates in chronic disease management, including psychosocial assessments;
• Document patient counselling appointments in the medical record. Minimum Qualifications : A Master's Degree (or higher) in a direct clinical practice Human Services field is required for this position.

Master's degree in social work, psychology, marriage and family counseling, marriage and family therapy, psychosocial rehabilitation counseling, psychiatric nursing, or very closely related field of study.
