Summary :

Reporting to the Clinical Manager of Home Primary Care and Palliative Care, will work at the top of their license and complete tasks that may include comprehensive nursing assessments, clinical triage, patient education, , acute assessments, disease specific care planning, quality improvement and support of value-based payment programs including chronic care management, principal care management, and Medicare's GUIDE model.

This Nurse Care Coordinator works with HomeMD and Palliative Care multidisciplinary teams to support patients and their families through transitional care needs across care settings (i.

e. acute care discharges back to community setting, hospice, etc.), and through disease stage changes in support of the care and / or treatment preferences established through goals of care discussions.

Qualifications :

  • Professional Registered Nurse currently licensed in Massachusetts.
  • Bachelor's degree required, master's degree in nursing, health care administration or related field preferred. One (1) to three (3) years of relevant experience, including clinical experience in hospice, home health, palliative care, geriatrics, or primary care experience with population health is preferred;

or, any equivalent combination of education and experience that provides the requisite knowledge needed to perform the essential functions of the job.

  • Experience with primary care case management with telehealth-based care models preferred
  • Experience working in a chronic care management program preferred.
  • Exposure to electronic medical records and point-of-care documentation.
  • Knowledge of Primary Care and Palliative Care operations and philosophy.
  • Demonstrated ability to provide supervision and evaluation to clinical staff.
  • Demonstrated ability to organize, prioritize and complete multiple tasks in a fast-paced environment.
  • Demonstrated empathy towards and ability to interact appropriately with patients of diverse social backgrounds, economic status, diverse cultures and individual lifestyles.
  • Excellent verbal and written communication skills with individuals and groups.
  • Demonstrated ability to communicate professionally and effectively in person, via telephone and via video conference.
  • Demonstrated comfort with and ability to use appropriate technology effectively. This may include accessing multiple electronic medical systems, patient care apps, video and / or telemonitoring equipment.
  • Must be a licensed driver with an automobile that is insured in accordance with state and / or organization requirements and in good working order.

The Pay Range listed is the anticipated hiring range of this position. Precise pay rates are determined through consideration of one or more of the following : education, training, relevant experience and work location.

There are some cases in which the pay rate could exceed the range listed on this job post. Pay Range

$41-$48 USD
