The role of the In-Home Care Physical Therapist involves evaluating and developing treatment plans for patients of all ages, modifying care plans when necessary.

This position also includes serving as a clinical resource for staff and students, and may require varying hours based on program need.

  • Document patient visits with comprehensive detail to ensure clear understanding of care given and plan for next steps.
  • Educate and document discharge planning, including communication with healthcare providers and coordination of outpatient visits and community resources.
  • Promptly update and close treatment goals as necessary, communicating and conducting case conferences as needed.
  • Coordinate care with the team and caregivers, providing written instructions and supervising assistants in the patient's home.
  • Administer various physical therapy treatments and assess patients to determine therapy needs.
  • Complete comprehensive assessments and documentation in compliance with regulations and policies.
  • Assess and address homebound status and safety of patients, recommending equipment and educating caregivers on home exercises.
  • Perform wound care as needed and within the scope of practice.
