We are looking for a Senior Primary Care Physician to join our rapidly expanding, physician-led and mission-driven primary care organization.
As a Senior Primary Care Physician, you will be responsible for providing care to patients with acute and chronic illnesses encountered in the older adult patient.
You will lead a care team consisting of a care promoter, care facilitator, and care coordinator, and will also engage with the transitional care team for patients unable to come to the office.
In addition, you will be accountable for patient care and outcomes, seek consultation from specialists when needed, and actively participate in the management of the center.
The ideal candidate will have a MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty, along with a current, active State Medical License and Board Eligibility.
