Summary :
The RN Inpatient Care Manager is a member of the care management team and works with the Hospitalist to improve the health of patients.
The RN will use the principles of care management, patient placement, and value-based care. The Care Manager will focus on care coordination and discharge planning for FMC Patients.
Essential to the role of a care manager is the ability to collaborate and participate in care coordination activities that meet the patient's needs.
These skills are essential to referrals to FMC's Ambulatory Palliative Care Program, ToCC, Total Care Program and Care at Home Programs.
The RN Inpatient Care Manager will incorporate both face-to-face and telephonic based services utilizing care management strategies.
The RN is responsible for working collaboratively with the patient, family, physicians, and the entire health care team.
The Care Manager will ensure that patients have the necessary support, resources, education, follow-up appointments and home care need to improve outcomes and reduce the risk of re-hospitalization.
This position applies the standards of practice and performance in the work setting to achieve positive outcomes and ensures the performed activities follow all applicable federal and state regulations.
Preferred Qualifications :
- Registered Nurse with a Florida License
- 3 years or more experience as an RN
- Knowledge and experience with the Medicare and value-based care population, including the benefits and services available.
- Has participated in discharge planning, collaborating, and coordinating health care services.
- Proficient in utilizing various electronic medical records (EMR)
- Able to work independently, manage a case load and complete assigned work on time.
- Attention to detail, strong organizational and time management skills.
- Effective communication skills- both written and verbal

Department Standards :
Performs department duties in a timely and efficient manner. Maintains the confidentiality of patient information.
Understands the role of the FMC Inpatient Care Manager RN and the purpose and goals of care management.
Represents FMC Hospitalist, Primary Care, and Care at Home Referring and Attending Physician coordinating, communicating, and collaborating with the patient, family, and various health care teams.
Prioritizes face-to-face visits based on specific patient needs, medical conditions, therapy progress, and social service interventions to facilitate the transitioning of patients to the next level of care.
Establishes a discharge plan with the patient, family, covering physician, PCP and collaborating teams upon admission to a facility using recommended DDA
Requests and reviews progress notes to reassess and reevaluate the discharge plan to best determine the appropriate level of care.
Communicates and coordinates with the health care team to identify obstacles and potential social barriers in the inpatient, community, and home setting.
Ensures patients have home care services arranged, such as, nursing, therapy, DME and community resources if appropriate.
Works to identify FMC leakage upon inpatient admission and coordinate transfer of service to FMC Hospitalist.
Responsible to share hospital readmission risk score with FMC stakeholders, such as PAC CM, TOCC, Ambulatory CM team, PCPs and specialists.
Responsible for escalating inappropriate admissions to IRF and LTACH
Works to identify and collaborate with patients, caregivers and FMC Providers to mitigate unwarranted inpatient admissions.
Works to identify and collaborate with patients, caregivers and FMC providers to refer to Care@Home, ToCC, inpatient and Ambulatory Palliative, Specialty follow-up appointments and CHF Program.
Completes all necessary documentation requirements efficiently and effectively inside Intergy.
Achieves internal goals and exemplifies the values, vision, and mission of Florida Medical Clinic.
Collaborates with patient, family and FMC providers to utilize informed choice process for patients requiring PAC services.
Performs other duties as assigned and is available to support other team members.
Ability to cross-cover for other team members and perform other duties as assigned.
