Summary :

The competent Nurse, in the same or similar clinicalsetting, practices independently and demonstrates an awareness ofall relevant aspects of a situation.

Provides routine and complexcare, in accordance with patient treatment plans. Continues todevelop the ability to cope with and manage contingencies ofclinical nursing.

Makes appropriate assignments and delegates toother care providers as a means to help manage the clinicalsituation.

CHRISTUS Santa RosaHospital - Westover Hills (CSRH-WH) is a 150-bed hospital servingthe fastest growing area of San Antonio.

Specialized care includesorthopedic and surgical services, ICU, women’s services, a newbornnursery, comprehensive cardiovascular care from diagnostics to openheart surgery, vascular lab, sleep center, emergency services, theCHRISTUS Weight Loss Institute, wound care, rehabilitation, andmore.

The campus also boasts an Outpatient Imaging Center and threemedical plazas, one of which houses our CHRISTUS Santa Rosa FamilyMedicine Residency Program and CHRISTUS Santa Rosa Family HealthCenter.

Responsibilities :

  • Meets expectations of the applicableOneCHRISTUS Competencies : Leader of Self, Leader of Others, orLeader of Leaders.
  • Meets expectations for flexibilityof assignment within the ministry.
  • Proactivelyprovide assignment availability on a regular basis.
  • Able to be assigned to more than one unit.
  • Consistentwith the ANA Scope and Standards of Practice, provides nursing careutilizing the nursing process, including assessment, diagnosis,planning, intervention, and evaluation for assigned patients.
  • Addresses increasingly complex psychological, emotional,cultural, and social needs of patients and families in accordancewith their level of practice.
  • Using the appropriateprotocol, administers medications and treatments; monitors forside-effects and effectiveness of the treatment prescribed.
  • Documents patient history, symptoms, medication, and caregiven.
  • Assess learning needs and provide education topatients, family members and / or caregivers; identify issues andresources.

Standard II. Patient Throughput& Patient Flow Process

  • Anticipates and plans foradmission / discharge / transfer needs to facilitate patientflow.
  • Utilizes appropriate systems of communicationand tools to facilitate the discharge process.
  • Coaches on tools and techniques for checking, cross-checking, andvalidating orders to ensure accuracy.

Standard III. Unit Operations

  • Plans, directs, andevaluates the overall nursing care and functions in a particularnursing unit during an assigned shift.
  • Demonstratesgood stewardship in the proper use and maintenance of equipment andSupplies.
  • Assesses departmental staffing needs;actively participates in resourcing efforts.
  • Educatesand trains others on the operations, ethics, and regulations withinthe industry.

Standard IV. SafePractice / Quality Care / Regulations

  • Incorporates patientsafety practices / guidelines to promote a safe environment,resulting in positive patient outcomes.
  • Demonstratesaccountability for nursing research and quality improvementactivities.
  • Provides evidence-based nursingcare.

  • Educates and trainsothers on the operations, ethics, and regulations within theindustry.

  • Transcribes verbal orders; explains techniques for ensuring theiraccuracy.

Medical Equipment

  • Describes experience with basic medical equipment used in own unitor facility.
  • Uses standard diagnostic tools andtechniques to resolve common equipment problems.
  • Educates patients about the appropriate use of home medicalequipment.
  • Ensures that all equipment and relatedsupplies are in proper working order prior to use to ensure patientsafety.
  • Inspects, troubleshoots, and evaluatesincoming equipment.

Medical OrderProcessing

  • Shares experiences with processing medicalorders for one or more groups of patients or conditions.
  • Describes functions and features of the system used toenter, validate, update, and forward medical orders.
  • Discusses common errors, their sources, and procedures forcorrecting.
  • Explains considerations for entering andfollowing standing orders.
  • Differentiates betweenstanding orders and preprinted orders and considerations foreach.

Patient Chart Reading andInterpretation

  • Describes experiences in reading andinterpreting patient charts for patients on the unit and under owncare.
  • Reviews patients’ charts for completion andaccuracy; identifies and alerts to mistakes or omissions.
  • Recognizes unexpected readings and alerts nursing ormedical staff.
  • Relates examples of misreadings ormisinterpretations and lessons learned.
  • Reviews,discusses, and validates own interpretation with others.

Patient Safety

  • Shares experienceswith ensuring safety for one or more patient groups orsettings.
  • Explains key features of safety guidelinesand procedures for those groups and settings.
  • Listensand responds to safety inquiries from patients and familymembers.
  • Recognizes and addresses physiological andpsychological signs of problems.
  • Describesconsiderations for patients who can cause harm to self, versus harmto others.
  • Utilizes appropriate systems to documentmisses and near misses, and participates in immediateinvestigating, analysis, and reporting in real time.

Requirements :

A. Education / Skills

Bachelor of Science Degree in Nursing,preferred.

B.Experience

  • Minimum of 2years of combined experience required in Medsurg andTelemetry.
  • 5 years of experiencepreferred.

C. Licenses,Registrations, or Certifications

  • BLS required.
  • ACLSrequired.
  • RN License in state of employment orcompact required.
  • PALS required within 30 daysof hire at CHRISTUS Children’s Hospital orSPOHN.
