Job Summary
As a Home Care Physical Therapist, you will provide therapy to patients based on their needs and medical direction. Additionally, you will educate patients and caregivers and arrange adaptive equipment as needed.
Essential Functions
- Perform evaluation procedures to assess disability levels and create appropriate treatment plans.
- Provide treatment programs and education to restore maximal function to patients.
- Document evaluation and treatment of each patient in compliance with department standards and medical-legal procedures.
- Instruct and train other rehabilitation staff, nursing, and medical personnel.
- Participate in ongoing process improvement, patient satisfaction initiatives, and program development
- Demonstrate productivity as indicated by department standards.
- Improve skill areas through continuing education and development of new concepts of patient care.
Qualifications
- Bachelor's Degree or equivalent from an accredited physical therapy program (Required)
- Preferred Master's Degree
- 1 year or more of relevant experience working within Home Care (Preferred)
- License - Physical Therapist - STATE MI State of Michigan (Required)
- Basic Life Support (BLS) - AHA American Heart Association or Basic Life Support (BLS) - ARC American Red Cross within 90 days of Hire (Required)
