Key Responsibilities

Conduct comprehensive physical therapy evaluations using diagnostic muscle, nerve, joint, and functional ability assessments.

Develop and implement individualized plans of care in coordination with physicians, focusing on rehabilitation goals and functional recovery.

Provide hands-on treatment to address impairments and improve mobility, strength, and independence.

Educate patients and caregivers on therapy plans, safety techniques, and proper use of mobility aids (canes, walkers, wheelchairs, prosthetics, orthotics, etc.).

Monitor progress and adapt treatment plans as needed; submit timely, accurate clinical documentation after each visit.

Collaborate with interdisciplinary care teams through case conferences and care plan updates.

Ensure adherence to safety standards and maintain therapy equipment.

Participate in ongoing education through staff meetings and in-service training.

Qualifications

Graduate of an accredited Physical Therapy program.

Current New York State Physical Therapy license in good standing (required).

Minimum of 1 year of experience as a licensed Physical Therapist, preferably in home health or outpatient settings.

Valid driver's license and reliable transportation (mileage reimbursement provided).

Excellent communication, time management, and patient education skills.

Schedule & Compensation

Full-Time | Monday-Friday | Day Shifts

Salary : $80,000 $96,000 / year (commensurate with experience)

Incentive Plan Available

Mileage Reimbursement included in each paycheck
