Responsibilities

Responsibilities :

• Implement and monitor patient care plans. Monitor, record and communicate patient condition as appropriate.
• Serve as a primary coordinator of all disciplines for well-coordinated patient care.
• Note and carry out physician and nursing orders.
• Assess and coordinate patient's discharge planning needs with members of the healthcare team.

Requirements

Licensure / Certification / Registration :

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Licensed Registered Nurse credentialed from the Texas Board of Nursing required.

Education :

Diploma from an accredited school / college of nursing OR Required professional licensure at time of hire.

Additional Preferences

Preferred Experience and Education :

• 2 years nursing experience
• Graduate from an accredited Wound Ostomy Continence nurse program or currently working on completion of an accredited wound ostomy continence nurse program
• Certified Wound Ostomy Continence Nurse (CWOCN) or Certified Wound Ostomy Nurse (CWON)
