Weekend Schedule Case Manager
This position has a schedule of Friday, Saturday, Sunday.
Clinical License required- RN, LSW, PT, OT, SLP - You must have a clinical license to be considered for this role
Compensation is hourly and ranges from $35 - $47 / hr based on experience.
Job Description
- Coordinate patient care from admission to post-discharge
- Manage resources and oversee interdisciplinary plan-of-care decisions
- Participate in planning for and the execution of patient discharge experience
- Facilitate team conferences and complete case management documentation
- Educate and communicate with patients and families
- Ensure compliance with regulations and participate in utilization review process
Qualifications
- Must have a clinical license and meet eligibility requirements for CCM® or ACM certification
- Minimum of an Associate Degree in Nursing for RNs, bachelor's degree for other eligible health care professionals
- 2 years of rehabilitation experience preferred
