• Possess RN licensure in the state of Michigan.· Possess a minimum of three (3) years of acute care / inpatient nursing experience.
• Inpatient Case Management and / or discharge planning and care coordination experience strongly preferred.· Can communicate effectively with patients, families, and hospital personnel, have strong critical thinking and creative problem solving skills and excel at planning.
