Description :

The Physical Therapist is responsible for delivering home health therapy services.

This role requires prioritization of start of care visits.

The therapist is required to supervise Physical Therapy Assistants.

Next day mandatory reporting of start of care visits is expected.

Clinical notes and other documentation must be submitted within 48 hours of the visit.

Maintaining timely communication with supervisors, intake, and office staff is essential.

The therapist should be responsive during normal business hours.

A valid California Physical Therapy License is required along with one year of Home Health experience.

The territory covered includes multiple counties, with potential for expansion as needed.

The schedule is full-time, Monday through Friday, with one weekend rotation per month.

Training consists of three full days, with one in-house and two in the field.

The documentation system utilized is Kinnser.

Equipment issued includes an iPad and mobile communication tools.

Mileage reimbursement is provided for travel between patient visits.

Productivity requirements include achieving 30 points per week, with specifics on visit types and point values outlined.

Overtime must be pre-approved by the Clinical Supervisor.
