Responsibilities
- Provide physical therapy services including assessment, evaluation, and goal-setting
- Update care plans as needed
- Submit completed visit notes and perform assessments as required
- Attend staff meetings and educational in-services
- Adhere to guidelines for patient care and discharge planning
- Ensure patient needs are met and individualized care plans are documented
Qualifications
- State Physical Therapy License
- One year of home care physical therapy experience preferred
- Ability to make sound clinical judgments
- Proficiency in clinical skills and Microsoft Office
- Valid driver's license and reliable transportation
