In this position, you will be instrumental in leading and managing the Social Services Department, while also providing direct social work services to residents and their families.

What You'll Do : Lead the Way : You will actively participate in planning, developing, implementing, evaluating, and overseeing our entire social services program.

Resident Advocate : Conduct thorough resident assessments, participate in discharge planning, and develop individualized social care plans that address each resident's unique needs.

Connecting with Care : Build strong relationships with residents and their families through interviews, providing social / emotional support, and connecting them with valuable community resources.

Empowering Others : Offer expert consultation to staff and community agencies on best practices for meeting residents' social and emotional needs.

Championing Confidentiality : Ensure the highest level of privacy and security for all resident information.Administrative Excellence : Contribute to developing and maintaining department policies and procedures, complete documentation, and assist in tasks such as arranging transportation and financial aid assistance.

You're a Perfect Fit If You Have : A Master's degree in Social Work (or a Bachelor's degree with extensive experience) and at least 1 year of supervised experience in a healthcare setting.

A valid Massachusetts Social Work License.Strong leadership skills with the ability to delegate tasks effectively.Excellent communication and interpersonal skills to build rapport with residents, families, and staff.
