Are you passionate about helping elderly and disabled adults? Victory Human Services is seeking a dedicated Care Manager to join our Adult Foster Care (AFC) program.
In this role, you will provide vital psychosocial and counseling support to elderly and disabled adults, ensuring their well-being in the AFC setting.
The Care Manager plays a key role in assessing care environments, developing care plans, and collaborating with nursing staff and caregivers to provide exceptional care.
This is a full-time position working 40 hours / week.
Requirements : Qualifications :
Qualifications :
- Bachelor’s degree and / or Social Worker license with 2 years of experience working with elderly or disabled adults, OR
- Bachelor’s degree with 2 years of clinical experience in elderly or disabled care.
- Language Requirement : Must speak Cape Verdean Creole and Haitian Creole.
Key Responsibilities :
- Conduct assessments and develop care plans for members.
- Select, train, and supervise AFC caregivers in collaboration with nursing staff.
- Perform regular on-site visits to ensure care standards are met.
- Respond promptly to client and caregiver concerns, and be available 24 / 7 for on-call support.
- Assist with accessing healthcare and community services.
- Maintain detailed care documentation and review safety procedures. Location : Areas in Brockton and Boston
