Description
This full-time position as an RN Case Coordinator involves working in a healthcare setting alongside an interdisciplinary team to assess and treat patients along the continuum of care.
The role includes developing and implementing discharge planning, collaborating with providers to optimize the use of acute care hospital days, and providing clinical leadership to healthcare teams to ensure patient care goals are met.
Qualifications
- Licensure as a Registered Nurse
- Three years of acute-care setting experience
- Knowledge of medical terminology and disease processes
- Excellent communication and negotiation skills
- Ability to work collaboratively with multidisciplinary healthcare professionals
- Preferably bilingual in English and Spanish
Education
- Completion of a formal Nursing program
- For Associate degree hires, a Nursing Baccalaureate Degree (BSN) must be obtained within 6 years of hire
Work Schedule
Full Time, 40 hours per week, Shift 1 (M-F 8a-4 : 30p), rotating weekends and at least 2 holidays per year
