Registered Nurse Home Visits Per Diem
Atrio Home Health is seeking a Registered Nurse interested in working on a per diem basis, conducting home visits. Opportunities are available in both Grand Rapids and Holland / Zeeland areas.
This team member has the flexibility to create their schedule with no weekend requirement. Ideally, working one to two shifts per week.
Responsibilities
- Routine visits - preventative nursing procedures and rehabilitative nursing procedures
- Admissions - initial assessments, establish nursing diagnosis, and develop / implement nursing care plan
- Wound care
- Client and family / caregiver education
- Documents accurately : OASIS, progress notes, and clinical notes
Location : Grand Rapids or Holland / Zeeland area
Schedule : Create your own schedule
Compensation : Our compensation model includes bonuses for high-quality client outcomes and efficient case management
Qualifications
- Licensed RN in Michigan; BSN preferred
- Valid Michigan driver's license, auto insurance, and reliable transportation
- 1-2 years of acute care or skilled nursing experience; prior certified home health experience preferred
- Passion for delivering high-quality care to older adults
