Position :  Family Medicine Physician, Emplify Health by Bellin

Location : Denmark and Wrightstown, Wisconsin

  • Full-time practice :

40 hrs / wk., 4 days / wk.

  • 28 hrs. minimum available for patient appointments
  • No weekend or holidays, except for on-call weekends
  • Call :  Phone only, participate in a large call group
  • 2-year compensation guarantee, plus :

    • CTO :

    160 hrs / yr. during guarantee

  • 264 hrs / yr. after 2yr guarantee
  • CME :

    • $5,000 / yr. and 80 hrs. of time off

  • Relocation Assistance :

    • Up to $12,000
