Job Description :
As a Licensed Vocational Nurse / Licensed Practical Nurse will provide direct patient care to residents in a long-term care setting.
Your role will involve administering medications, monitoring patient health, and collaborating with the healthcare team to ensure the highest quality of care for residents.
Responsibilities :
- Perform comprehensive assessments of residents’ health and well-being.
- Administer medications and treatments as prescribed by physicians.
- Monitor and document vital signs, changes in condition, and responses to treatments.
- Assist residents with activities of daily living (ADLs) and provide emotional support.
- Collaborate with registered nurses and other healthcare professionals to develop and implement individualized care plans.
- Educate residents and families about health management and care procedures.
- Adhere to safety protocols, infection control practices, and facility policies.
Qualifications : Education :
Education :
Graduate from an accredited LPN / LVN program.
Licensure :
Current New York State LPN license.
Experience :
Previous experience in long-term care or a similar healthcare environment preferred.
Skills :
- Strong clinical skills, excellent communication abilities, and compassion for patients.
