Sunbelt is seeking a Registered Nurse for a Home Health contract position with a local client near Lewisburg, WV. Home Health RNs with at least one year of recent home health experience are encouraged to apply for this opportunity.

Employment Type : Full-time, 13 week contract with potential to extend
Works Hours : 8 : 00AM-5 : 00PM, 40 hours per week, some on call and weekends required
Responsibilities :
- Utilize expert knowledge of OASIS Assessment Documentation Start of Cares / Admissions, Recertifications, Resumptions of Care, Transfers, and Discharges
- Complete accurate and detailed OASIS Documentation within a 24-48 hour turnaround time
- Case manage at least 5-7 patients per day and 25-30 patients per week
- Scheduled visits will include, but are not limited to :
- Providing wound care
- IV administration and teaching
- Cardiac program follow up and intervention
- Travel in theLewisburg, WVarea
Experience :
- At least one year of Home Health experience within the last year required
- In depth knowledge and in-field experience of OASISrequired
- Start of Care / Admissions experience required
- Experience case managing at least 5-7 patients per day and 25-30 per week
Education :
- Associates Degree of Nursing or Bachelors Degree of Nursing
- West VirginiaRegistered Nurse License
- American Heart Association Basic Life Support
