- Proficiency in OASIS and HomeCare HomeBase EMR systems within the last year
- Reliable transportation with a current driver's license and liability insurance
- Willingness to travel up to 75 miles from the home clinic
Job Description :
- Coordinate and direct the provision of home nursing care according to physicians' orders and agency policies.
- Supervise and evaluate Home Health Aides and Licensed Practical / Vocational Nurses.
- Conduct home visits and periodic office meetings.
- Document and travel between 5-10 patient visits per day, managing cases such as Start of Care, Wound Care, and Follow-up Visits.
- Engage in on-call rotations as required, including weekends once every 5-7 weeks.
- Cover the designated regional territory, with an expectation of traveling approximately 150-200 miles per week.
- Attend to productivity expectations by achieving 30 points per week through various types of visits and activities.
