Job Summary :
The RN Care Manager is responsible for performing comprehensive assessments on patient admissions and facilitating the care coordination process.
The role focuses on high-risk patient populations to ensure appropriate discharge planning and prevent readmission. The RN Care Manager collaborates with healthcare professionals and communicates vital patient information to ensure optimal care outcomes.
Essential Functions and Responsibilities :
- Understand and adhere to WPH Performance Standards, Policies, and Behaviors.
- Complete a comprehensive initial assessment for all patients.
- Coordinate multidisciplinary care both within and between hospitals.
- Conduct transitional planning activities, including patient / caregiver education and aftercare arrangements.
- Engage with commercial payers and address high-utilizer patient needs.
- Communicate and collaborate with health team members to relay relevant patient information.
- Ensure department goals related to Length of Stay (LOS) reduction, readmission prevention, denial reduction, and satisfaction improvement are met.
- Document patient assessments and care plans as per department requirements.
- Participate in Performance Improvement activities as needed.
- Perform Utilization Review (UR) activities in accordance with department policies, including :
- Commercial payer reviews
- Disposition determination
- Retro / self-audit
- MC variance
- Admission clinical review
- Collaborate with healthcare professionals to evaluate and ensure the safe discharge of geriatric patients, including inpatients, emergency department patients, and surgical outpatients.
- Perform any other related duties as assigned.
Qualifications :
- Registered Nurse (RN) NYS license required.
- BLS certification required.
- COVID-19 vaccination required.
- PRI certification to be obtained within three months of initial employment.
- Screen certification strongly preferred.
- Minimum of three years of clinical experience in Emergency Department, critical care, or medical / surgical settings preferred.
- Previous case management experience preferred.
- Knowledge of healthcare financial, regulatory, and payer issues preferred.
- Understanding of state, local, and federal programs preferred.
- Experience using Milliman / InterQual criteria preferred.
- Strong computer skills are essential.
