PRIMARY DUTIES AND RESPONSIBILITIES :
- Manage assigned caseload by providing intensive case management, and facilitating linkages and communication with stakeholders.
- Define programs or interventions to modify individual dynamic risk factors as identified by a systematic assessment process.
Address the risks and needs and build on the identified strengths of each individual. Provide a long-term road map to achieve continuity in the delivery of treatments and services and sharing of information both across and between agencies.
Works in the community to expand capacity with services such as housing, employment, substance abuse, mental health, transportation, etc.
in order to ensure identified needs of supervisees are met and the risks of committing a new crime are mitigated.
- Coordinates and communicates DCS reentry goals and initiatives; facilitates the process of open communication and effective coordination to ensure community input is captured and expectations for implementation are clearly defined.
- Responsible for facilitating and executing the communication strategy; serves as a liaison between local community stakeholders and the Department of Community Supervision to ensure that reentry initiatives address community concerns and overall response.
- Coordinates meetings, focus groups and other local events to ensure effective two-way communication channels are established and maintained at all levels within the community.
- Partners with the local community leaders, landlords, and other stakeholders to identify, access and retain community assets and services that can provide a seamless transition for individuals returning from prison, thus improving their chances for success.
- Proactively identifies and recommends actions to mitigate the impact of policy and operational barriers among state and local agencies and critical support channels that are essential for both program and client success.
PREFERRED QUALIFICATIONS :
Applicant should meet at least three or more of the following : Completion of a Master's degree in a related field; Two years or more experience in program monitoring and compliance;
Two years or more of partnering with community and civic groups, community and civic leaders, agency leaders, public officials and the public to gain support for program initiatives.
Two years or with strategic planning, implementation, community organizing, etc. and Two years or more experience working in a human service delivery area.
Open to internal (DCS employees) and external candidates.
Travel throughout the circuit is involved.
Bachelor's degree in a related field from an accredited college or university and four (4) years of professional job-related experience which includes one (1) year in a lead worker, team leader, or supervisory role;
or Two (2) years of experience at the lower level Social Svcs Prgm Consultant 2 (SSP041) or equivalent position.
