As a Physical Therapist inour clients Home Health team you will be responsible for providingPhysical Therapy services to patients in their homes assessingpatient functioning levels and developing treatment programs toimprove independence and physical / cognitive functioning.
- You willalso be involved in patient and caregiver education supervision andreporting on patient progress. Key Responsibilities PerformPhysical Therapy assessments and develop treatment programs Recordand report patients reaction to treatment and condition changesInstruct patients and caregivers in therapy programs and device useSupervise physical therapy assistant and when appropriate thecertified nursing assistant Prepare reports and participate instaff and Agency development activities Schedule and Shift DetailsMondayFriday;
- 8 : 00a / 9 : 00a5 : 00p 1 weekend per month (Saturday& Sunday; day shift w / scheduled pat. ients); 2holidays (1 summer / 1 winter);
