The Licensed Practical Nurse is anessential part of the team responsible for providing anddocumenting skilled nursing care in accordance with the developedcare plan and physicians orders for each individual patient whileadhering to confidentiality standards and professional boundariesat all times.
Essential JobFunctions
Deliverquality patient care in compliance with physician orders underdirection of their Registered Nurse teammate.
Perfom thecontinuum of hands on client care through documentation and timelycommunication with the client's physician and othercaregivers
Recognizes changes in patient needsand responses requiring intervention and implements care to preventrisk or reduce risk.
Educate clients and their familymembers based on clients specificneeds.
Provide care in patients home using avariety of skills such as phone triage patient educationobservation / assessment wound care infusions catheter care PICC linedressing changesetc.

Requirements :
- Anactive LPN License in the state ofapplication
- Valid CPR
Preferred :
- MedicareSkilled Nursing experience
- Basic understandingof Oasis
- 1year LPN experience in a home healthor health care setting
- Home Care Home Base expaplus
