Highlights
Community Physician Network is seeking a physician to join our outpatient practice at our East Health Pavilion on the east side of Indianapolis, IN.
Well-established, outpatient only practice
Shared call schedule with 24 / 7 RN triage
East Health Pavilion also houses Family Medicine, Pediatrics, Dermatology, full lab services, X-ray and PT and rehab
Patient Centered Medical Home (PCMH) practice model with pharmacist, dietician and care navigation nurse
Electronic Medical Record system (Epic)
Licensure & Certifications
Board certified / board eligible in Family or Internal Medicine
Licensed in Indiana or eligible for medical licensure in Indiana
