About The Position
Job Title : Physical Therapist - Home Health Job Specialty : Physical Therapy Home Health Job Duration : 13 weeks, with an option for a 26-week contract Shift : Day 5x8-Hour (8 : 00 AM - 4 : 30 PM) Guaranteed Hours : Facility can call off up to one shift per pay period if needed Experience : 1 year of skilled home health experience required License : Pending license accepted Certifications : BCLS Must-Have : - Primary series COVID vaccination plus one booster- Oasis proficiency Job Description : Provide skilled physical therapy services in home health settings.
Manage a caseload with a 1 : 1 ratio, visiting up to 6 patients per day.- Utilize EPIC for charting and documentation.- Drive to client's homes;
occasional maximum radius of over 50 miles.- No weekend or on-call duties required.- Must have personal transportation.- Facility groups client visits to minimize drive time.
Float between facilities within the state as needed.- Wear any color scrubs as attire.- Open to first-time travelers.- Local candidates are welcome and considered within a 25-50 mile radius at the same rate.
