Possession of a current license topractice as a registered nurse in the state of Connecticut.EXPERIENCE : One (1) year of recent experience in communityhealth / homecare as a nurse.

Provides intermittent nursing service to patients andtheir families in compliance with state and federal regulations,incorporating the following : A complete physical and psychosocialassessment.
Establishment and implementation of an appropriate careplan and goals with the patient / family. Referral to appropriatetherapies as needed.
Periodic reevaluation of significant changesin the condition of the patient and of progress toward the goals.Use of Agency and community consultants / resources.
Instruction ofpatient / family in procedures or responsibilities they are assumingand optimum wellness practices. Planning for the patient dischargefrom service.
May admit / readmit patients to service, as requestedby the Supervisor of Clinical Services. Death pronouncements.Manages work assignment in effective and efficient manner : Documentation is complete, accurate and exported within 36 hours.
Reports any cases to the Weekend Manager where goals cannot be met,or at-risk situations exist and responds appropriately.
Remainsflexible in accepting cases on a p.r.n. basis or unexpected visits.Utilizes payer sources correctly, reflecting changing levels ofcare.
Identifies and utilizes community resources for optimalpatient care from case # manager. Orders appropriate medications,Durable Medical Equipment and Supplies for HomeCare patients perestablished practices.
Coordinates with weekend manager to addresschanges in the plan of care / need for additional services.Demonstrates an understanding of OASIS : OASIS documentation isconsistent with the Plan of Care.
More than 75% of OASIS data isanswered accurately and in accordance with OASIS walk. OASISassessment and Plan of Care are consistently reflected andsupported in documentation greater than 75% of the time.
Developsand maintains sufficient professional skills and credentials tomeet the needs of patient care : Attends a minimum of 12 hours ofin-service per year, to include mandated annual in-services.
Participates actively in self-appraisal and the formation of a planof self-development. Knowledgeable of Agency policies andprocedures.
Seeks appropriate supervision. Projects professionalimage through attitude and compliance with dress code. Functions asa team member with consideration to all staff.
Provides Agency withcurrent copy of professional license. Maintains CPRcertification.
