$1,000 Sign-On Bonus

This position has Hybrid capabilities for those who reside in or around Ottawa area

We are looking for a full-time Aftercare Case Manager to add to our team! All you need is a bachelor's degree in human services and a valid driver's license.

A Day in The Life as a Case Manager

As an Aftercare Case Manager, we provide support and services to children and families that have achieved permanency through reintegration, adoption, custodianship / guardianship, or aged out youth.

During this work with post-permanency placements the focus is on risk management for their designated aftercare period. This can include crisis intervention for the child / family when necessary, working closely with the public and private agencies providing supportive services, maintaining accurate and timely documentation / reports, coordination of community / educational services.

Aftercare Specialists meet with the child / family as determined by the aftercare plan, this is in-person meetings to support the reunification and address or resolve concerns.
