QUALIFICATIONS :
Graduate from an accredited school of nursing, current licensure with State Board of Nursing Examiners.
One year of geriatric nursing experience preferred.
Must be able to speak, read, write the English language.
JOB SUMMARY : In accordance with facility policies and procedures, the Licensed Practical Nurse provides clinical evaluation of the direct care to residents that is of high quality and under the supervision of the Nurse Manager and DON.
Receives input from GNAs and communicates timely with the RN Staff Nurse to meet resident care needs.
DUTIES AND RESPONSIBILITIES :
Addresses resident care needs within the LPN scope of practice. As needed, seeks assistance from the resident's physician, Nurse Manager, ADON or other RN member of the Nursing leadership team.
Within the scope of the LPN practice, completes ordered resident treatments and medications timely and accurately.
Addresses resident care needs and / or resident or family concerns and involves the Nurse Manager, ADON or members of the leadership team and needed.
Works collaboratively with co-workers and members of all departments caring for the residents.
Completes assignments timely, thoroughly, and accurately with sensitivity.
Participates fully in the Quality Assurance and Performance Improvement activities of the organization.
