Schedule :

Once you complete your orientation, your schedule as a Palliative Care LPN will typically be Monday to Friday, 8 : 00 AM to 5 : 00 PM.

There are no weekends or call associated with this role.

What you’ll be doing :

  • We provide quality care regardless of circumstance to a variety of different socioeconomic and cultural backgrounds.
  • As a Palliative Care LPN working in our clinic you’ll collaborate and coordinate with external medical staff and referral sources, as well as our own clinical teams for continuity of care.
  • You’ll be assisting field staff with, or independently completing, prior authorizations for medications prescribed by palliative care.
  • Assisting with rooming patients which includes vital signs, medication reconciliation, basic assessment of chief complaint and related documentation.
  • Along with clinic duties, you’ll be calling patients for symptom checks, assisting patients with refills, phone triage, or other patient questions or concerns.
  • You’ll need to be comfortable educating the patient and caregivers. This might include how to identify signs and symptoms, medication information, and plans of care.
  • You will be part of an Interdisciplinary Team (IDT) consisting of a physician, nurse practitioner, registered nurse, and medical social worker.
  • You’ll be cross trained with clinic scheduling and other administrative tasks.

Job Specifications :

  • Education : LPN : Graduate of an approved school of practical nursing.
  • Licensure / Certifications : Current license as a practical nurse in the State of North Carolina / South Carolina.
  • Experience : Two years’ experience required. Office / clinic, palliative care or oncology experience preferred.
