Licensed Practical Nurse (LPN) for United Hospice - Orange County, NY - Full / Time, Day Shift

Salary for this position is $26.00 / hr. to $34.00 / hr. and is commensurate with applicable experience. Title : Licensed Practical Nurse - LPN
Location : Primarily Orange County, NY, but you may be asked, from time to time, to provide care to patients in Rockland County, NY.
Schedule : Full / Time position, Monday through Friday with a weekend weekend rotation.
Job Description Summary :
As an LPN in our community program, you are an integral member of the clinical team providing essential care and support to the patients under the supervision of a Registered Nurse.
Qualifications :
- Graduate of an accredited practical nursing program
- Possession of a valid and current NYS LPN license
- Minimum of 2 years’ experience in nursing with recent clinical experience in either hospice or hospital-based med / surg nursing
- Complies with accepted professional nursing standards and practice
- Committed to the Hospice Philosophy of Care
- Ability to relate to a diverse population and workforce in a personable and effective manner
- Able to communicate effectively while sustaining a caring, kind and professional disposition
- Must have excellent written and verbal communication skills
- Electronic medical record competence is required
- Admission and end-of-life care experience is preferred
Responsibilities of the LPN will include, though is not limited to :
- Must be comfortable providing patient care in a homecare setting such as : homes, apartments, assisted living facilities, skilled nursing facilities, etc.,
- Actively participates in the coordination of all aspects of resident care in accordance with current professional nursing standards and practice
- Able to provide direct care according to the established resident plan of care
- Documents in the EMR services rendered as outlined on the resident plan of care in a complete, accurate and timely manner
- Contributes to resident and family counseling and education
- Provides appropriate pain and symptom management according to the resident’s plan of care
- Keeps the RN apprised of the resident’s condition as needed
- Adheres to all policies and procedures established by United Hospice
- Uses equipment and supplies appropriately
- Maintains resident confidentially at all times
- Participates in ongoing interdisciplinary comprehensive assessment
- Accepts other responsibilities as assigned
