Are you a Nurse Practitioner looking for a side job with flexible hours? This per diem role allows you to create your schedule while earning supplemental income and working independently.

Position Overview : Nurse Practitioner Health Risk Assessments

As a Nurse Practitioner, you will perform comprehensive health risk assessments for Medicare Advantage members in their homes.

This position focuses on evaluating patients' physical, mental, and social needs without the responsibility for ongoing care.

Key Responsibilities :

  • Conduct detailed in-home health risk assessments for Medicare Advantage members.
  • Identify and document all current and chronic health conditions, assessing physical, mental, and social needs.
  • Provide patient education and counseling on identified conditions and screening procedures.
  • Record assessment findings using a provided iPad no prescriptions, follow-ups, or on-call duties required.

Requirements :

  • Active and unrestricted Nurse Practitioner license.
  • Minimum commitment of 18 hours per week, including one full 8-hour day, M-S.
  • Ability to work independently and autonomously.
