Company Overview : Care Net Home Health is a sister company to our Angel Care Hospice & Palliative agency in Arcadia, CA.
We are a patient-centric organization whose goal is to promote quality home health services throughout Los Angeles County.

Responsibilities :
- Direct all daily patient referral and intake operations including providing oversight of the establishment and implementation of intake policies.
- Conduct comprehensive assessments of patients' medical history, current condition, and treatment goals.
- Coordinate and monitor patient care, ensuring that all necessary medical services and resources are provided.
- Monitor and evaluate the effectiveness of treatment plans and make adjustments as necessary.
- Ensure compliance with all state, federal, and CHAP referral / intake regulatory requirements.
- Contribute to the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy.
- Establish and maintain positive working relationships with current and potential referral sources.
- Ensure seamless transition of patients to home care by providing coordination of patient education and preparation for home care, plan of care initiation, and coordination of care with multiple service providers.
- Ensure maximum third-party reimbursement through oversight of insurance verification and authorization processes.
- Gather and report referral statistics including key customer referral trends.
- Contribute to strategic planning including identifying opportunities for additional or improved services to meet unmet customer needs.
- Maintain comprehensive working knowledge of community resources and assist referral sources in accessing community resources.
