About The Position
Job Title : Licensed Master Social Worker Job Specialty : Care Coordination and Social Work Job Duration : 59-day assignment Shift : 8 : 30 AM - 4 : 30 PM, Monday to Friday (Possible holidays required) Guaranteed Hours : 0 Experience : Hospital Social Work experience and Case management experience License : Licensed Master Social Worker (LMSW) by the State of New York Certifications : Master’s Degree in Social Work (MSW) Must-Have : Robust discharge planning and multitasking abilities- Familiarity with EPIC EMR- Proficiency in Allscript referral management and transportation setup- Strong computer navigation skills- Experience with psychiatric / pediatric population- Ability to collaborate and interact with an interdisciplinary care team Job Description : Provide effective care coordination and social work services within a healthcare setting.
- Conduct comprehensive discharge planning and implement efficient case management practices.- Utilize EPIC EMR for documentation and patient care management.
- Manage referrals and transportation setups using Allscript systems.- Navigate computer systems efficiently for record-keeping and communication.
- Work closely with psychiatric and pediatric populations, applying specialized knowledge and skills.- Collaborate with the interdisciplinary care team to ensure continuity and quality of patient care.
