Responsibilities
In this role, you will be responsible for :
- Completing initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs.
- Providing professional nursing care using all elements of the nursing process.
- Developing and implementing care plans based on nursing diagnosis and palliative nursing actions.
- Administering medications and treatments as prescribed by physicians.
- Assisting patients with activities of daily living and promoting self-sufficiency.
- Coordinating patient care as a Case Manager when assigned.
- Communicating with physicians, community health personnel, and interdisciplinary group members.
- Maintaining accurate clinical notes and records.
What We're Looking For : We are looking for candidates who possess the following qualifications :
- Registered nurse with a current license.
- Graduate of a National League for Nursing accredited school of nursing.
- Current CPR certification.
- Minimum of one (1) year of nursing experience within the last three (3) years OR a baccalaureate degree in nursing from an accredited program and a current RN license.
- Strong observation, problem-solving, and communication skills.
- Ability to work independently and prioritize workload.
- Experience in public health, home care, or hospice nursing preferred but not required.
- Management experience not required.

- Counseling patients and families on nursing and related needs.
- Educating patients on self-care techniques and providing health counseling.
- Coordinating patient care as a Case Manager.
- Attending interdisciplinary group meetings.
- Participating in on-call duties and ensuring necessary equipment and services are available.
