$1666 upon completion of 30 days, $1667 upon completion of 6 months, $1667 upon completion of 1 yearWe are seeking a new PACT Nurse for our Program for Assertive Treatment (PACT) teams to provide treatment to adults with persistent mental illness.

The PACT Nurse will work on an interdiciplinary team of nurses, clinicians, and outreach workers to assist those in need in the community.

The PACT Nurse will work under the supervision of the Lead Nurse to conduct psychiatric assessments in addition to physical health needs in order to provide holistic nursing services.

The PACT Nurse will be responsible for making appropriate referrals to community physicians, providing management and administration of medication in addition to providing a range of treatment, rehabilitation, and support services.

Other responsibilities include but are not limited to : * Providing case management for an assigned group of persons served, including coordinating and monitoring the activities of the individual treatment team* Conducting a comprehensive assessment of psychiatric history (e.

g., onset, course and effect of illness, past treatment and responses, and risk behaviors), mental status, and diagnosis;

Additionally, this position requires a fully insured vehicle that you will use in the course of your daily work. You must be willing to transport clients in your vehicle.

  • Must be able to successfully pass a CORI, reference, multi-state background, professional licensure and driving record check.
