Key Responsibilities :

• Provide compassionate, patient-centered care , ensuring our residents feel as cozy as an autumn evening.
• Administer medications and treatments prescribed by healthcare providers with precision and care.
• Monitor and assess residents' health , updating patient records to reflect any changes just like the changing leaves.
• Collaborate with a multidisciplinary team to develop individualized care plans that meet the unique needs of each resident.
• Maintain a safe and clean environment for residents, following infection control protocols as diligently as prepping for cooler days.
• Communicate effectively with patients, families, and healthcare team members, fostering a warm and supportive environment.

Qualifications :

• Current LPN license in the state of Ohio.
• CPR certification .
• Experience in skilled nursing or long-term care settings is preferred.
• Strong clinical assessment and critical thinking skills as sharp as the fall breeze.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a fast-paced environment and adapt to the changing demands, much like the beautiful fall season.
